The most depressing thing about an illness is the sense of being left out or brushed aside by it from the mainstream of living. When a person is ill, he cannot keep up with the world of healthy people. Life begins to occur without him. Even language seems to change. What is said to a sick person does not involve him, and therefore he begins to interpret it differently. He loses touch with the outer, healthy world. His sense of isolation is increased rather than lessened by the regular visits of the doctor. The doctor labels the sick person’s disease and confines him to his special place. With tact and skill, the doctor completes the patient’s helplessness and makes final his division from the world of strong arms, sound legs, clear skin, and lungs that breathe deeply. The sick person’s only hope is to return to life again, and he clings to the thought of getting well.

1 Increasing numbers of American doctors are using a technique known as hypnosis. They say hypnosis often can help persons suffering pain and stress. It also can help speed the healing of bums, and treats some forms of asthma and some skin diseases. Hypnosis is not new. It has been used for many years both in scientific research and to please crowds at public gatherings,
2 Hypnosis is commonly described as a condition similar to sleep. But, experts say it is more a form. of deep thought. The thought becomes so intense that it is longer just a thought. To the per son, it becomes reality.
3 Hypnotized patients are reported to have increased self-control and a reduced sense of pain. Some doctors use hypnosis to limit pain during a medical operation. Hypnosis is used mainly when the patient may have problems with usual anesthetic or pain-killing drugs.
4 Experts say there is little chance that a patient will awaken during such an operation. But, if this happens, the operation is temporarily halted, and the patient hypnotized again. Doctors may advise hypnosis for women who are giving birth. Dentists may use it in place of traditional pain-killing drugs, such as novocaine (麻醉药).
5 Hypnosis also has been used to treat burn victims. Researchers have found that bum victims who are hypnotized improve faster that those who are not. But, they are not sure why this happens.
6 Hypnosis can reduce or end a patient's pain. But experts say this does not mean the patient is cured. The problem that caused the pain still must be treated.
7 Experts also say persons cannot be forced to do something they would normally oppose. That is why hypnosis often is not effective in treating cigarette smokers and persons who eat or drink too much. The success of the technique depends on how much someone wants it to succeed.

These conditions continued until after World War 1I. At that time, new treatments were discovered for some major mental illnesses theretofore considered untreatable (penicillin for syphilis of the brain and insulin treatment for schizophrenia and depressions), and a succession of books, motion pictures, and newspaper exposes called attention to the plight of the mentally iii. Improvements were made, and Dr, David Vail's Humane Practices Program is a beacon for today. But changes were slow in coming until the early 1960s. At that time, the Civil Rights Movement led lawyers to investigate America's prisons, which were disproportionately populated by blacks, and they in turn followed prisoners into the only institutions that were worse than the prisons the hospitals for the criminally insane. The prisons were filled with angry young men who, encouraged by legal support, were quick to demand their rights. The hospitals for the criminally insane, by contrast, were populated with people who were considered "crazy" and who were often kept obediently in their place through the use of severe bodily restraints and large doses of major tranquilizers. The young cadre of public interest lawyers liked their role in the mental hospitals. The lawyers found a population that was both passive and easy to champion. These were, after all, people who, unlike criminals, had done nothing wrong. And in many states, they were being kept in horrendous institutions, an injustice, which once exposed, was bound to shock the public and, particularly, the judicial conscience.
Judicial interventions have had some definite positive effects, but there is growing awareness that courts cannot provide the standards and the review mechanisms that assure good patient care. The details of providing day-to-day care simply cannot be mandated by a court, so it is time to take from the courts the responsibility for delivery of mental health care and assurance of patient rights and return it to the state mental health administrators to whom the mandate was originally given. Though it is a difficult task, administrators must undertake to write rules and standards and to provide the training and surveillance to assure that treatment is given and patient rights are respected.
